RESUMEN

Pancreas transplantation has been successful for restoring euglycemia in persons with type 1 diabetes mellitus. The transplantation may be performed as a single procedure, or it may follow or be performed simultaneous with kidney transplantation. This article discusses potential consequences of hyperglycemia and hypoglycemia and indications, alternatives, and contraindications for transplantation. The author also describes postoperative nursing care and potential complications including surgical and immunologic. Potential outcomes of pancreas transplantation can include survival benefit, physiologic improvement, and enhanced quality of life.

RESUMEN

People who receive a kidney transplant experience overall better quality of life (QOL) than those on dialysis. However, one area of QOL that does not seem to improve after transplant is sexuality. The purposes of this review were to evaluate the literature specific to sexuality after kidney and kidney/pancreas transplant, identify gaps in the literature, and suggest directions for future research. Sources for this review included studies published in nursing and medical journals as well as studies from the related fields of social work and psychology. This literature review revealed several factors that may affect posttransplant sexuality and sexual functioning including chronic illnesses, pretransplant dialysis, and medication side effects. In addition, numerous studies have shown that problems with sexual functioning are prevalent in the transplant population affecting up to 50% of kidney transplant recipients. These problems have a strong negative association with QOL and life satisfaction. Future research should focus on identifying the specific sexual concerns of kidney transplant recipients and on the development and testing of interventions to help alleviate these concerns.

Neste estudo, discorre-se sobre a experiência do enfermeiro com os pacientes no pós-operatório de transplante de pâncreas em um hospital de grande porte de Belo Horizonte. As autoras apresentam a construção de um protocolo assistencial para essa clientela, bem como relatam as ações do enfermeiro nas áreas administrativa, assistencial, de ensino e de pesquisa, privativas do enfermeiro.


The present study describes the nursing experiences during the postoperative period of patients who underwent pancreatic transplant in a large hospital of Belo Horizonte. The authors present the elaboration of care protocols for such patients and report the nursing actions in management, care, teaching and research.


El presente estudio enfoca la experiencia del enfermero en el posoperatorio de pacientes con trasplante de páncreas internados en un gran hospital de Belo Horizonte. Las autoras presentan la construcción de un protocolo asistencial para estos pacientes y también relatan las acciones del enfermero en las áreas administrativa, asistencial y de investigación, particulares del enfermero.

RESUMEN

This paper explains the development of protocols to aid in the care of patients following a combined kidney/pancreas transplant. With adequate planning of care, medical and nursing problems can be detected at an early stage thus ensuring immediate and appropriated action. We have found protocols to be invaluable in the care of these patients and the one which we developed on fluids and electrolytes is outlined in this paper.

RESUMEN

Nursing care across the service delivery continuum is essential for patients undergoing simultaneous kidney pancreas transplantation. The self-management of complicated treatment choices and care regimens requires education and support, which are the domain of transplant nursing coordinators. Even successful transplantation is associated with complications in the first 6 months. Continuous posttransplant nursing interventions to monitor patients, teach and reinforce self-care, and support adjustment is crucial to patient well-being. This article describes nursing strategies used at one transplant center to address these patient needs.

RESUMEN

Diabetes is the leading cause of end-stage renal disease in the United States. Combined kidney and pancreas transplantation is a safe and effective treatment option for diabetic nephropathy. During the past decade, pancreas transplants had improved outcomes as a result of improvements in pancreas recovery and preservation, the surgical procedure, immunosuppressive regimens, and immunologic monitoring. Current 1-year patient and graft survival rates are 90% and 80%, respectively, and evidence is accumulating that improvements occur in microvascular and neuropathic complications as well. Successful outcomes of kidney and pancreas transplantation are due in large part to careful nursing assessment, diagnosis, intervention, teaching, and discharge planning.

This article summarizes the historic and current perspectives related to pancreatic and islet cell transplantation. The nursing care of patients who are recipients of these types of transplantations is described. The complications and risks for these types of transplantations along with the most common surgical and medical regimens are also described.

RESUMEN

Pancreas transplantation in the type I diabetic patient offers hope for a normoglycemic state and arrest, or in some cases, reversal, of secondary complications of diabetes. Rejection in the pancreas graft can be difficult to detect, but much work is being done on different methods to recognize rejection. Patients report increase in psychological well-being after pancreas transplantation. Because of the benefits, it is a viable option for treatment of the patient with type I diabetes.

RESUMEN

Combined kidney-pancreas transplantation is a safe and effective treatment option for patients with end stage renal disease (ESRD) resulting from type I diabetes. Current 1 year graft survival rates are nearing 80% and evidence is accumulating that improvement occurs in microvascular and neuropathic complications of diabetes after transplantation. This article is a detailed overview based on the current literature and our experience at The University Hospitals of Cleveland of the challenges and benefits of kidney-pancreas transplantation and the nursing care required to prepare the patient for home.

RESUMEN

For patients with severe diabetes complicated by renal failure, simultaneous kidney-pancreas transplantation offers some hope. The National Dialysis and Kidney Transplantation Study reports that transplant patients have a higher quality of life, higher employment, and higher perceived health beliefs than patients on dialysis. With this thought in mind, simultaneous kidney-pancreas transplantation can be seen as an appropriate and attractive alternative. That is, simultaneous kidney-pancreas transplantation is a viable option for the patient with type I diabetes mellitus and ESRD.
